Form Structure Elements

Form Structure Elements are Editable by the form builder and Non-editable by the form User, They are used to build the structure of the Form.

Section – The main container for grouping the form Data Elements

  • Alignment - Left/Middle/Right Heading display

  • Repeatable answers – Allows a User to Repeat the fields of the Section on the form by pressing a [+] button

  • Display as table in PDF - Data elements display as tiles (up to 3 columns) in the PDF's Section rather than as a list

Heading – Adds a Heading to the Form or Form sub-section

  • Alignment - Left/Middle/Right Heading display

Link – Adds a URL to the form that a User can “click” on to be taken to a website

  • Default Value - Embeds the entered URL into the forms displaying field “Label”

Paragraph – Adds non-editable text to the form. I.E. for form instructions

Image – Add an image or images to the form

Divider – Adds a line on the Form. I.E. to give a visual separation of Sections or Data Elements

PDF Blank Space – Creates a blank area on the PDF view only, this allows writing/drawing on the printed PDF

  • Height – Select the number of lines the PDF Blank Space will display

Form Data Elements (Editable by the form User)

Form Data Elements are Editable by the form User. They are used by the form User to enter data into the Form

Text – A form User can enter text or numbers into this form field

  • Description – Displays non-editable text under the text entry field to the User

  • Height – Sets the number of text lines the User can enter

  • Column Span (PDF Only) – Displays the PDF as columns 1, 2 or 3 columns

  • Required – The form User must make an entry in this form field to be able to Save the form

  • Read-only – Makes the form field a Read-only field to the User

Date/Time – A form User can enter/select a Date or Time to this form field

  • Mode – Displays a Calendar Picker or Time Picker in this form field

  • Required - The form User must make an entry in this form field to Save the form

Number – A form User can enter numbers only into this form field - The field displays a - / + button so the field can be used as a Counter

Single/Multi Select – Radio buttons or Tick Box fields on the form

Photo – A form User can up-load a photo to this form field

Signature – A form User can enter a signature to this form field, either manually or if have a signature uploaded to Katipolt Desktop, insert that signature into the form field
